Coach Evaluation Forms 2020-21 Season
Each season, and in keeping with our efforts to provide the best possible experience for all of our players, coaches and volunteers, Chilliwack Minor Hockey (CMHA) would like to hear back from the members and provide you with an opportunity to evaluate the Head Coach as well as the Assistant Coaches of your player's team.
CMHA's goal is to provide coaches with positive/constructive feedback that will encourage them to continue coaching again next season.
CMHA respects the privacy of every submission. Every submission, including any personal information you provide, will be kept strictly confidential and is protected by confidentiality agreements signed by all involved with the Coach Selection Committee. Submissions will be reviewed by the Coach Selection Committee and your input will only be used in combination with the responses of others participating in the survey. Submissions will be stored securely by CMHA.
